Portage DD is pleased to announce a one-time opportunity to apply for financial assistance to support participation of persons served by Portage DD in connecting with the community through a variety of experiences.
Examples include, but are not limited to:

Amusement Parks
-
Concerts
-
Theater Performances
-
Sporting Events
-
Lessons, Trainings, Workshops, Conferences

Day and Overnight Trips
To apply for funds, follow these instructions:
1. Complete information below, download application below, or pick up a paper copy of the form at the Happy Day Services Building, 2500 Brady Lake Road, Ravenna, Ohio 44266, Monday through Friday between the hours of 8am-3:00pm. Completed forms can be dropped off during the same hours or mailed at that address to the attention of Michelle Dolensky.
2. Committee will review application and notify of status. Applicant will be notified within 10 business days after submission.
3. If approved, select one of these options:
a. Submit receipts for reimbursement with remittance information
b. Submit detailed invoice and receive check for authorized amount prior to event.
4. Send a picture of you having fun to Portage DD to events@portagedd.org
DEADLINE TO APPLY IS OCTOBER 31, 2024
