School-Age Survey — The Portage County Board of Developmental Disabilities

Portage DD is trying to assist families with their preschool and school-age children, ages 3 to 21. We realize there are some unmet needs for them and we would like to first identify what the most important needs are. Please rate the ten items from 1 to 10 with 1 being the most important need. We realize that we may not be able to do all of these, but we know we can do more and will.

Please take the time to answer our survey. Your input will help us determine what types of services/programs are most important for your child. We want to develop something that is meaningful for families.
